Yes, the NCAA and the University require documentation of sickle cell screening results. View State Contact Info & Testing Dates
What is my athletic ID #?
Your Athletic ID# is your Student ID# assigned by the University. Use only your official school ID number.

Insurance card photo size?
Photos must be less than 1 MB. To reduce size, lower the resolution or crop the image.
Note: If you have Willamette Insurance, upload a photo of your ID (Drivers License) until your card arrives Aug 1.
Physicals & Doctor Visits
Incoming athletes and transfers can schedule physicals on or after June 1st. Bring your completed Health History Form and the Official Physical Form. Only a surgeon or specialist may sign return-to-play forms for surgeries.
Who takes the ImPACT test?
Required for all first-year and transfer athletes. Football players are required to take it every year.
